5. 의사에 의한 침 시술

 

*침술 관련 법제도

• 미국의 의사는 1800년대 초부터 침술을 수행해 왔다. 많은 주에서 의사위원회가 침을 실시하는 의사의 관리를 행하고 있는데, 의사가 침을 실시하기 위한 요건을 요구하는 침술위원회를 설치하고 있는 주도 있다
• 각 주의 의사위원회 등에서 의사의 침술에 관한 조사가 2016년부터 2017년에 걸쳐 실시되었다. 그 결과는 아래표와 같다. 대부분의 주에서는 의사가 업무범위 내에서 침술을 실시하는 것이 허용되고 있지만, 3개 주에서는 비의사와 동일한 침술면허 취득을 요구하고 있다. 또한 의사가 침술을 실시하기 위해 추가 교육을 요구하는 주도 있다. 일부 주에서는 의사가 침술을 실시하기위한 규칙이 없다 ( Lin K, Tung C. 5-1 미국의사침술학회 The American Academy of Medical Acupuncture (AAMA)

1987년 설립, 서양의학에 침요법을 포함하는 북미의 의사 전문학회. 
1300명 이상의 침 임상을 한 의사(physician acupuncturists)가 회원. 국제 회원도 있음

AAMA는 침술의 교육을 받아 침술의 능력을 갖춘 의사가 실시하는 침을 medical acupuncture라고 부르고, 과학적 근거에 근거한 침과 의료의 통합을 진행시키는 것을 목적으로 하고 있다 .

 

*정회원은 다음의 요건을 충족시킨다.
 1) 미국 또는 캐나다의 현역 의사면허 보유자.
 2) 의료안에서 침 시술을 포힘한다.
 3) 최소 220시간(120시간 강의, 100시간 임상연수) 이상 정식으로 의사를 위한 침술교육의 수료.

5-2 미국 침술의학 인정 의학회  American Board of Medical Acupuncture(ABMA)
미국의사침술학회(AAMA)로부터 발전 독립된 조직으로 2000년에 창립되었다.
미국 침술의학 인정의학회(AMBA)의 사명은 안전하고 윤리감을 가진 효과적인 medical acupuncture를 국민에게 보급하는 것이며 이를 위해 침 임상에 종사하는 의사에게 전문의로서의 높은 수준을 유지한 시험으로 인정한다.
medical acupuncture란 '전통적인 침술을 현대의 생명의과학을 기반으로 하는 의료로 통합한 포괄적 지식을 핵심으로 하는 의학분야'로 규정. 영역이다.

5-4 의학으로서의 침술

미국에서 침 임상을 실시하는 의사의 수자는 불분명하지만 의사를 대상으로 한 침술을 임상에 도입하기 위한 교육 프로그램(HMI's Medical Acupuncture for Physicians program)을 6000명 이상의 의사가 수강했다고 보고되고 있다.

*Mayo Clinic(Mayo Clinic)의 침술 운영 현황

 

메이요 클리닉은 직원 65,000명, 연간 수익 140억＄를 기록한 학술 의료 시스템의 비영리법인. 5개 주에서 운영되고 있으며 미국뿐만 아니라 약 140개국에서 연간 100만명 이상을 케어하고 있다. US News & World Report에 의해, 로체스터는 2020~21의 전미 제일의 병원으로 선택되고 있다

 

메이요클리닉 매사추세츠종합병원 등 일류 병원에서는 침술사를 직원으로 고용하고 있다. 2008년부터 침술 연구 프로그램에 미국 정부의 비용 7300만 달러를 지불했으며 이때 매사추세츠 종합병원은 보건사회복지부로부터 2600만 달러의 침술 연구의 재원 지원을 받았다.

2006년 7월 1일부터 2011년 3월 31일까지 사이에 미네소타주 로체스터의 메이요 클리닉의 CAM치료에 관한 현황(침, 마사지를 받은 환자수, 침술을 받은 환자의 진단 등)은 아래 그림/표와 같다. 침술은 침술 교육을 받은 의사 또는 침면허 보유자에 의해 이루어졌다

 

 

 

*Boston Children’s Hospital의 침술 진료

 

보스턴 소아 병원은 40개 이상의 임상 부문과 258개의 전문 임상 프로그램이 있는 미국 최대 소아 의료센터 중 하나이다. US News and World Report에 의해, 전미 제1의 소아 병원에 랭크되었다. 하버드 메디컬 스쿨의 주요 소아 교육병원으로 연간 2억 2500만 달러의 연구 자금을 모아 1,100명이 넘는 과학자가 연구를 실시하고, 희귀한 질병이나 복잡한 상태의 어린이들의 치료도 실시하고 있다

 

보스턴 소아 병원의 마취학, 구명 응급 및 통증 치료 부문의 한 부분에 Medical Acupuncture Service가 있다. Medical Acupuncture Service에는 침술 교육을 받은 의사가 기존 서양의학 치료와 침술을 통합하여 급성·만성 통증이나 통증과 관계없는 질환을 관리하고 있다 .

 

Medical Acupuncture Service에서는 생후 3주 신생아에서부터 침술을 적용하고 있으며, 자궁내막증, 소아암, 소아의 섬유근통증과 근골격계의 통증, 두통, 겸상적혈구증 등에 적용되고 있다

본 시설은 소아의료의 지도적 역할을 담당하는 시설로 U.S. News & World Report의 소아병원 랭킹으로 3년 연속 전미 NO1이 되고 있다.
침의 주된 대상은 두통, 복통, 하지통, 등통, 흉부통인데, 여기에 더해 소아는 암, 낫상적혈구병, 혈우병, 낭포성 섬유증, 자궁내막증, 클론병도 대상으로 하고 있다.

침 치료는 침의 특별한 교육을 받은 서양의학 면허를 보유한 의사들에 의해 이루어지고 있다.

5-9 미국 의사와 침술사의 만성 요통침 치료방법 비교
"Comparison of physician and nonphysician acupuncture treatment for chronic low back pain" 중에서

[배경]미국의 1만며명 침 시술자의 1/3은 의사이지만 면허를 가진 침(뜸)사와 비교하여 의사의 침술 임상방법에 대해 거의 알려지지 않았다. 본 연구는 비의사의 침술과 비교하여 의사의 침술을 분석한 최초의 연구이다.

[방법] 직업별 전화번호부, 미국침술의사학회(AAMA) 회원명부, American College of Graduate Medical Education(ACGME) 인정 진찰소 명단 등 3가지 정보원을 바탕으로 무작위 추출한 의사에 의한 침 시술자 464명에게 질문지를 2000년에 송부하였다. 응답수는 137, 응답률은 37%였다. 답변을 비의사인 워싱턴주 면허를 가진 침(뜸)사에 대해 보고된 유사한 조사연구 데이터와 비교하였다.

[결과]의사는 신경해부학적인 방법, 프랑스의 기 치료, 이침, 중의학, 경피적 전기신경 자극 등 다양한 진료 방법을 도입하여 침을 시행하였고 침치료 부위의 선택에는 신경해부학적인 방법을 중시하였다. 대조적으로 침(뜸)사는 침치료 부위의 선택은 중의학의 방법으로 하였다. 의사와 침(뜸)사는 침 진료 방법이 확연히 다른데도 불구하고 요통의 침 치료에 이용하는 경혈은 거의 같았다. 침치료 외에 의사는 진통제나 트리거포인트 주사 등 다른 의학적 치료를 하고 있었는데, 침(뜸)사는 침과 더불어 뜸, 마사지 등 중의학적 다양한 방법을 시행하고 있었다.
